As a Care Coach, you will be the interface providing training, guidance, and support to the family caregivers we hire. You should have an exceptional service and compassion mindset. This is a work-from-home job with periodic in-person visits across GA required.
Assist clients and primary family caregivers in determining eligibility for assistance and resources within the Medicaid Waiver program.
Conduct family-centered assessments and identify support needs for caregivers to help provide care at home.
Provide ongoing health coaching, training, and support to family caregivers to ensure client goals are met.
Maintain effective communication with primary family caregivers, registered nurses (RNs), and Medicaid waiver case managers.
Monitor family caregivers' daily progress notes and tasks performed via the Givers mobile application.
Conduct monthly supervisory calls and semi-annual in-person case conferences with clients, caregivers, RNs, and care coordinators.
Prepare and manage client files with a focus on accuracy and compliance with regulatory standards.
Collaborate with the Agency Operations team to ensure caregiver files are compliant with regulatory standards and assist with caregiver qualifications and documentation.
Build collaborative partnerships with local community organizations to promote the benefits of the Givers program.
Represent the organization at occasional provider, advocacy, and partnership meetings.
Perform other duties as assigned.
Minimum two (2) years of experience in home health, Medicaid waiver programs, or similar fields, preferably working with elders or people with disabilities.
A valid driver's license and a car to travel to the homes of member families.
Bachelor's degree in social work or an equivalent combination of education and experience in related fields.
Strong communication, relationship building, assessment, and problem-solving skills.
Proficiency with case management systems and mobile applications.
Ability to work independently and maintain effective communication channels with all stakeholders.
Knowledge of community resources related to medical, housing, financial, and social issues for clients' care.
Compassionate, understanding, and able to handle stressful situations with empathy and professionalism.
Valid driver’s license and automobile insurance, with the ability to travel within assigned geographic areas.
Strong organizational skills and meticulous attention to detail for compliance and regulatory standards.
Proficient in Zendesk (or other CRMs), Google Drive, and other software tools.
